Step 2: Submit Documents
As part of processing your admission to Minnesota State College Southeast, we will need prior academic transcripts and an Immunization Form.
Academic Transcripts
All undergraduate students will be required to submit ONE of the following documents before they can register:
- An official high school transcript (even if you have taken courses at other Minnesota State colleges or universities) - OR-
- A GED diploma - OR -
- An official transcript from a college or university that is not part of the Minnesota State system.
Please note: A signature and a fee may be required by the school prior to their mailing out a transcript. High school transcripts do not need to include a graduation date, as some students may still be in high school and will not graduate until June.

College Transcripts
If you are transferring credit from other colleges, request official transcripts to be sent to Southeast. An official transcript is a transcript that typically bears the signature of the registrar and the official institution seal, and is sent directly from the issuing institution. If you previously attended a Minnesota State college or university, we will pull your transcript automatically once you have been accepted. You do not need to submit one.
Check with your sending institution to see if e-delivery is possible. This is generally quicker than postal mail and will help speed up the transfer evaluation process. Ask your institution to email the transcript to: [email protected]. Otherwise, college transcripts can be sent by mail to:

Immunization Form (eForm)
Fill out the Immunization Form online (eForm).
If you graduated from a Minnesota high school before 1997, or you are a student from outside the state of Minnesota, please provide evidence of immunization on Part 3.
Please note: Some programs of study may have additional health requirements.
